Individuals older than age 12 vaccinated at the clinics will receive a $50 gift card to Pete’s Fruit Market or Monterrey Market at those sites. Individuals younger than age 12 vaccinated at the clinics will receive a $10 gift card.

WHO: Community members ages 5 and older needing first, second or booster COVID-19 vaccinations. All eligible individuals will be vaccinated regardless of ID, documentation or insurance status. Children ages 5-17 must be accompanied by a parent or legal guardian.

WHAT: Children’s Community Health Plan (CCHP) in partnership with Health Connections is planning several upcoming COVID-19 mobile vaccination clinics. Free COVID-19 testing is also available.

WHERE: Clinics will be held at the following dates and locations:

10 a.m. to 2 p.m. Thursday, Dec. 9 at Pete’s Fruit Market – Northside

2323 North Doctor MLK Jr. Drive, Milwaukee
10 a.m. to 2 p.m. Friday, December 10 at Pete’s Fruit Market – Southside

1400 South Union Street, Milwaukee
11 a.m. to 3 p.m. Monday, December 13 at Monterrey Market

3920 South 27^th Street, Milwaukee
