We want to support you and your family by making the COVID-19 vaccine available in your community. Visit one of our clinics below and earn a gift card. Individuals 12 and older who receive a vaccine or booster at a CCHP vaccination clinic will get a free $50 gift card. Individuals ages 5-11 will get a $10 gift card. These clinic events are open to the entire community, and vaccines are available for all eligible individuals. The COVID-19 vaccine saves lives.

WHO: Community members ages 5 and older needing first or second COVID-19 vaccinations; community members ages 12 and older needing booster COVID-19 vaccines. All eligible individuals will be vaccinated regardless of ID, documentation or insurance status. Children ages 5-17 must be accompanied by a parent or legal guardian.

WHAT: Children’s Community Health Plan (CCHP) in partnership with Health Connections is hosting several upcoming COVID-19 mobile walk-in vaccination clinics. Free COVID-19 testing is also available.

WHERE:  Clinics will be held at the following dates and locations:

  • 10 a.m. to 3 pm. Monday, Jan. 17 (Martin Luther King Jr. Day)
    Mark Quality of Life Center, 1641 West Atkinson Ave., Milwaukee
  • 10 a.m. to 2 p.m. Tuesday, Jan. 18
    Holy Cathedral COGIC, 2677 North 40th Street, Milwaukee
  • 10 a.m. to 2 p.m. Monday, Jan. 24
    Canaan Missionary Baptist Church, 2964 North 11th Street, Milwaukee
