This study is looking at the link between one’s neighborhood and CKM syndrome – or heart and kidney disease – in Milwaukee. Additionally, it will assess how social determinants influence risk factors and healthcare access for Milwaukee adults with CKM syndrome.

YOU MAY BE ELIGIBLE IF YOU

Are 18 or older
Have any of the following: diabetes, pre-diabetes, hypertension, hyperlipidemia, kidney disease, heart disease, stroke, or a BMI of 25 or higher
Living in Milwaukee County
